Our complete range of services and supports to the adult population are delivered at home. Many of the adults we serve have chronic illness or are recovering from surgery or hospitalizations. There is no place like home to recover from an illness, surgery, or injury. At HSC, we are dedicated to providing a full range of services delivered to your home in order to help you maintain your independence even in times of need.

You will find our staff to be exceptionally well trained and carefully screened. Above all, our staff are professionals with empathy, compassion and understanding of the importance of emotional well being in addition to physical health.

Our professional team can:

• Provide a nursing assessment to evaluate needs and develop a specialized plan of care with client, family, physician and other healthcare professionals.
• Provide nursing care.
• Provide physical, occupational, speech, and respiratory therapies.
• Provide dietary consultation.
• Assist with daily activities.
• Provide case management.
• Coordinate the billing of third party payers such as HMOs, Medicare, Medicaid or Commerical Insurance companies.
